Palliative Care in the Eastern Metro Region

Palliative Care in the Eastern Metro Region of Melbourne is provided where people need it. For some people it is at home (private or residential); for others, it is in hospital. Many health workers provide care using a palliative approach. Specialist teams provide professional advice and support to health workers and for families in caring for their loved ones.

Eastern Health: provides hospital-based care as well as consultations for staff providing care in residential care homes. Hospice care is based at Eastern Health's Wantirna site.

Eastern Palliative Care: the specialist community-based palliative care service for the Eastern Region of Melbourne with services offered in the local government areas of Boroondara, Manningham, Maroondah, Whitehorse, Monash, Knox and Yarra Ranges. EPC provides specialist palliative care to people with a life-limiting/terminal illness with complex symptoms whether they be physical, psychosocial, emotional or spiritual.

St.Vincent’s: provides hospice care at Caritas Christi in Kew. St Vincent’s Palliative Care Services is dedicated to providing the best possible healthcare, drawing on the talents and creativity of staff and volunteers. 

Bolton Clarke: provides support at home with a range of home nursing, home assistance, allied health and mental health services. 

Eastern Melbourne Primary Health Network (EMPHN): one of 31 primary health networks across Australia. EMPHN works with stakeholders to improve the primary health care system in eastern and north eastern Melbourne by increasing the efficiency of medical services, reducing fragmentation of care and improving health outcomes for everyone, especially for the most vulnerable. 

North Eastern Melbourne Integrated Cancer Service (NEMICS): a clinical network focussing on cancer-related health care.  NEMICS is one of nine Victorian Integrated Cancer Services (VICS) working in partnership with health services to facilitate coordinated and high-quality cancer care, system integration and collaboration.
